Kuroba Kaito was born a girl.
This, in fact, is pretty much the only change; Aoko is a girl, and the others are the same as they always were, but even so Kaito grew up loving magic just as much, and lost her father at the same age, in the same way.
And so, eight years later when a Kaitou Kid copycat started making the news and she swore to Aoko that her magic was better, and if that was the case then she would defeat and bring down the Kaitou Kid, when she found the secret room in their house later that night full of costumes and gadgets...
She took up the costume, binding her chest just as she did with any other male disguise, and put the mask upon her face.
With the last, she was 'Kuroba Kaito, girl magician' no more. At least, not for that night, nor many others to come. She had become 'Kaitou Kid'. And as all knew, Kaitou Kid was male.
Her performances were so good that she fooled almost everyone; apart from, of course, for a rare few exceptions - Hakuba Saguru, who flirted with her constantly even as Kid after he realised that she was female from a strand of hair; Koizumi Akako, who controlled the hearts of all men but one, since the Kaitou Kid was no man, and she held a bitter grudge against Kaito for allowing her to lose face in such a way... and, possibly, Kudo Shinichi, whose deductive reasoning (and impossible thought it might seem, differing perspective due to being several feet shorter) lead him to the conclusion.
Kaito is a rather tomboyish girl; she uses 'boku' rather than the feminine 'watashi', which is more polite than her male counterpart's use of 'ore' yet is about the same level of 'rudeness', due to the change in gender.
She wears her hair in a similar way as Kaito in canon; shorter hair makes it easier to wear wigs, and is also harder to catch on things.
She dresses casually, neither boyishly nor overly feminine. She can appreciate things being cute, but she's just as likely to go around in whatever her canon self would wear as a skirt or dress.
